The Nurse Manager is a member of the Community Living Center, and is qualified by education and experience to provide administrative oversight for the Green House Project. In alignment with this responsibility, knowledge of and experience related to the following departmental programs is essential: Community Living Center (CLC). He/she posses intimate knowledge of Long-Term Care federal regulations, VHA Guidelines, medical review criteria, clinical documentation requirements, current standards of care and Green House compliance Guidelines. He/she will perform as the FHCC Nurse Supervisor providing oversight and management of nursing practice as the Chief Nurse of the Day representing the FHCC Nurse Executive as needed. The Nurse Manager (NM) is responsible for staffing the Households (HH) on a continual basis (24/7), validating resident acuity, and working with other Nurse Managers to identify and share resources. The Nurse Manager participates in unit/facility committees (and task forces) to improve nursing practice and resident care and identifies educational needs of staff and ensures that these needs are addressed. He/she has an integral role in supporting and facilitating student clinical experiences, as well as orienting new Nurse Managers.
The Green House model is aligned within the Inpatient Geriatric Services Division, and is a program of the Community Living Center. The Green House model is based on the premise of a de-institutionalized effect that restores individuals to a home setting with the VA community, maximizing both privacy and positive experiences of living in a community. The NMs leadership style and direction maybe denoted as the "Guide" as an expected practice for FHCC small homes through the trademark of "The Green House Project". The model combines small homes with the full range of high level personal care and skilled nursing clinical services expected in high-quality nursing homes. The Green House Project offers an innovative national model for long term care that returns control, dignity, and sense of well-being to veterans, their families, and direct care staff.
Demonstrates leadership and mastery of practice in delivering efficient, comprehensive, and continuous resident-driven holistic care through active collaboration, communication and coordination of resources. The Nurse Manager executes leadership that is characterized by the implementation of strategies to serve as coach and mentor for the three most complex aspects of the Green House Model - Empowered Staff, Self-Managed Work Teams. The Nurse Manager collaborates with managers from other Divisions as needed to routinely review and evaluate performance for those directly supervised by others. The Nurse Manager acts as a direct supervisor to the self-managed work team of Shahbazim (Health Technicians), Nursing Assistants (NA), RNs, Licensed Practical Nurses (LPNs), and other assigned staff and is responsible for evaluating the outcomes of their work. He/she promotes shared responsibility in staff development and coordination of training.
The Nurse Manager serves on local, VISN and National committees and workgroups as needed to support the mission of the Green House project. She/he also works closely with stakeholder groups, including Green House liasons, volunteer organizations, and other interested parties. The Nurse Manager independently seeks individual learning opportunities to improve knowledge about the Green House model of care and related concepts.
